Specifications include, but are not limited to: The County of San Diego is seeking proposals from firms interested in providing Countywide HIV Services: Minority AIDS Initiative Bridge Program. The purpose of the Minority AIDS Initiative Bridge Program (Bridge Program) is to identify People of Color who are HIV-positive and out of care to provide linkages and support to engage them into the full range of available HIV Outpatient Ambulatory Health Services (OAHS), treatment and prevention services. The Bridge Program is intended to augment the efforts of HIV OAHS providers and Medical Case Management providers to reengage patients into care and maintain them in care over time. As a result, Contractor shall be a provider of HIV OAHS and Medical Case Management. Additionally, Contractor shall have capacity to support mental health services, substance abuse treatment and other supportive services with other funds or documented collaborative agreements to assure a fully integrated, comprehensive program.
