Specifications include, but are not limited to: In coordination with the Epidemiology Unit within the Epidemiology and Immunizations Services Branch of Public Health Services, Contractor shall provide contact tracing and community health worker services as-needed to support community disease control activities for COVID-19 and other infectious diseases. Other infectious diseases include diseases and conditions reportable to the health department via local, state, or federal reporting requirements. Contact tracing and community health worker engagement support the public health aim to reduce infectious disease transmission in the population by tracing and monitoring close contacts of confirmed cases and utilizing field staff to connect through home/field visits with vulnerable and hard-to-reach populations that have not been reachable by other means.
