A. Evaluation Planning and Design 1. Review the LICN Logic Model (Attachment B), Measures & Goals (Attachment C), and CMSP reporting templates (Attachments D-F) to ensure alignment with required evaluation metrics and deliverables. 2. Develop a written Evaluation Plan, to be approved by KCDPH, outlining proposed methods, indicators, data sources, data collection tools, and analysis strategies. 3. Define evaluation questions, process measures, and outcome metrics that correspond to LICN goals and measures, in alignment with CMSP reporting requirements. 4. Coordinate with KCDPH to establish baseline data, reporting frequency, and timelines for all required deliverables, including Quarterly Data Summaries, CMSP Progress Reports, and the Comprehensive Evaluation Report. 5. Identify how evaluation findings will inform the development of KCDPH’s Performance Management and Quality Improvement (QA/QI) Plan utilizing the PHAB template framework (Attachment A) or another evidence-based performance improvement framework identified in consultation with KCDPH. The proposer shall also include, if applicable, opportunities for staff education or training to support implementation and ongoing use of the QA/QI framework. If training represents an additional cost, it shall be clearly itemized in the cost proposal. B. Data Collection and Management 1. Assess existing KCDPH and partner data systems (e.g., REDCap, referral tracking systems) for accessibility, completeness, and data integrity. 2. Recommend and implement data-quality improvements to enhance accuracy and consistency. 3. Collect and assess quantitative data on program reach, service utilization, and outcomes, and qualitative data from clients, CHAs, and partners in alignment with evaluation goals and CMSP reporting. 4. Maintain comprehensive data documentation to support accurate and timely preparation of required reports (e.g., Quarterly Summaries and CMSP Reports). 5. Ensure full compliance with Health Insurance Portability and Accountability Act (HIPAA) and County data-security standards, and support KCDPH in developing internal dashboards or monitoring tools. C. Data Analysis and Interpretation 1. Perform descriptive, comparative, and trend analyses of quantitative data to assess progress toward LICN goals and measures, and to inform CMSP reporting. 2. Conduct thematic analysis of qualitative data to identify participant experiences, barriers, and program strengths. 3. Synthesize findings across data sources to inform CMSP Biannual Reports, the Sustainability Plan, the Comprehensive Evaluation Report, and the Final Grant Report. 4. Prepare Quarterly Data Summaries, at a minimum, and provide preliminary recommendations to guide ongoing quality improvement and program decisionmaking. 5. Present findings and recommendations at least quarterly, or more frequently as determined in coordination with KCDPH, through written reports, visual summaries, and formal presentations (e.g., PowerPoint or oral briefings) for program leadership,
