The Community Health Equity & Promotion (“CHEP”), a branch of the Department of Public Health (“City” or “DPH”), is requesting grant proposals from qualified agencies to provide the City with a Community Health Worker (“CHW”) and Medi-Cal Reimbursement (“CalAIM”) Pathways Program. This initiative will award two grants of $160,000 each to community-based organizations that can implement a CHW-driven model to improve health outcomes and build the infrastructure needed for long-term Medi-Cal billing. This program is being offered as a grant to support community-led innovation, organizational capacity-building, and culturally responsive service delivery. Grants allow for flexibility and collaboration, which are essential for testing new approaches and learning from community experience. Through this pilot, DPH seeks to strengthen frontline health services by investing in organizations with deep ties to San Francisco’s most impacted communities. The program also aims to help organizations develop sustainable, reimbursable CHW services.
