Specifications include, but are not limited to: 1) Contractor shall ensure the drug testing laboratory be secured at all times. Sufficient security measures shall be in place to control access to the premises and to ensure that no unauthorized personnel handle specimens or gain access to the drug testing laboratory processes or to the areas where records are stored. Access to these secured areas shall be limited to specifically authorized individuals whose authorization is documented. 2) Contractor shall ensure the drug testing laboratory use the “Chain of Custody” procedures set forth by the United States Department of Health and Human Services Substance Abuse and Mental Health Services Administration (SAMHSA) for all testing procedures. An approved “Chain of Custody” form shall be used for maintaining control and accountability of each specimen from the point of collection to receipt by the drug testing laboratory, through completion of testing, reporting of results, during storage, and continuing until final disposition of specimen. The date and purpose shall be documented on an appropriate “Chain of custody” form each time a specimen handled or transferred for every individual. Contractor shall provide CHP confirmation of the “Chain of Custody” procedures prior to the commencement of the Agreement for review and approval. In the event “Chain of Custody” procedures are not followed, this Agreement may be terminated at the discretion of the CHP. 3) Contractor shall be responsible for the following: a. Provide toxicology testing and “Chain of Custody” procedures to account for the integrity of each urine specimen by tracking its handling and storage from point of specimen collection to final disposition of the specimen. b. Provide the approved “Chain of Custody” form that shall be used for maintaining control and accountability of each specimen from the point of collection to final disposition of the specimen. c. Provide upon request, from the contracted physicians listed on Attachment I, Physician and Clinic Locations, or CHP Project Representative, the collection kit that shall consist of all materials normally used in specimen collection procedures; i.e., urine container, seal for container, temperature strip, label, and packaging containers and supplies. d. Provide shipment of collected specimens to the drug testing laboratory using overnight courier services. The specimens shall be placed in containers designed to minimize the possibility of damage during shipment. 4) Contractor shall be responsible for any expense related to overnight courier services, supplies for the “Chain of Custody” forms, postage costs, drug testing laboratory reports and all other direct and indirect costs associated with this Agreement. 5) CHP will not provide sugpport services for this Agreement. Contractor shall be responsible to provide all staff and services necessary to perform these services.
