Specifications include, but are not limited to: Contractor shall provide the County of San Diego Health and Human Services Agency (HHSA), Medical Care Services (MCS), and other County departments, including Self-Sufficiency Services, Behavioral Health Services, Sheriff’s Department, Probation Department, and the Public Safety Group with administrative support, facilitator services, and project management. Contractor shall assist with identifying current processes, protocols, and Information Technology (IT) systems, and map out gaps and needs to create recommendations necessary for an expanded pre-release Medi-Cal enrollment and suspension processes in the County’s adult and youth detention and reentry facilities.; 7.1. Meeting Preparation and Project Management: Contractor shall participate in pre-meeting preparation, virtually or in-person, with MCS prior to meeting with other County departments. Contractor shall be responsible for the following, including but not limited to: 7.1.1. Coordinate and schedule meetings; 7.1.2. Create meeting agendas; 7.1.3. Draft meeting notes; 7.1.4. Create flow charts and process maps; 7.1.5. Create PowerPoint presentations; 7.2. Meeting Facilitation: Contractor shall be responsible for the following, including but not limited to: 7.2.1. Facilitate and/or co-facilitate meetings between MCS and other County departments; 7.2.2. Review agenda, and get approval for previous meeting notes and other topics as required; 7.2.3. Solicit feedback and comments when appropriate from meeting attendees; 7.2.4. Seek out agenda items for next meeting and summarize any action items; 7.3. Notetaking: Contractor shall take meeting notes and develop a meeting summary report that includes action items, questions asked, and answers provided. Summary report shall be provided to the Contracting Officer’s Representative (COR) no later than seven (7) business days after meeting.; 7.4. Process Flow Development: 7.4.1. Contractor shall document current workflows and processes by creating flow charts and process maps using Visio or similar software.; 7.4.2. Contractor shall document current policies and processes and identify those that work well, as well as identify gaps and needs in the current processes, areas for improvement, and suggest process redesign to improve and expand the current processes.; 7.5. Information Technology (IT): 7.5.1. Contractor shall have knowledge and experience with local, State, and federal data sharing regulations and best practices for the exchange of Personally Identifiable Information (PII), Criminal Offender Record Information (CORI), and health information among human services agencies, justice agencies, government agencies, and social service programs. This includes information derived from the California Law Enforcement Telecommunications System (CLETS).; 7.5.2. Contractor shall document current data systems and data sharing practices used in the current pre-release Medi-Cal enrollment and suspension process between County departments.; 7.5.3. Contractor may be requested to provide recommendations on IT solutions and information exchange technology between County department to enhance the pre-release Medi-Cal enrollment and suspension process.; 7.6. Funding Application: 7.6.1. Contractor shall assist with drafting the Department of Health Care’s CalAIM PATH Justice Involved Capacity Building Round 2 funding application.
