The County of San Joaquin is soliciting Request for Proposals (RFP) for its PUBLIC HEALTH SERVICES Department. The County of San Joaquin, Maternal, Child, and Adolescent Health Program (MCAH) is issuing this Request for Proposals (RFP) to solicit proposals from interested and qualified organizations for the provision of maternal and child health services to Black pregnant and parenting women (16 years and older), their infants from birth to 12 months, and their families within San Joaquin County for the Black Infant Health (BIH) Program. This RFP establishes guidelines, criteria, and procedures for submitting proposals. The MCAH program expects to contract with One organization that successfully demonstrate the ability to provide culturally and linguistically appropriate community education and care coordination services that are designed to promote healthy lifestyles, provide access to quality maternal and child health services for pregnant and parenting Black women and reduce infant mortality per the guidelines described here-on. The services to be provided under this RFP are for three years.
