Specifications include, but are not limited to: a) Implement behavioral health teams who will respond to behavioral health needs across all HSH-managed permanent supportive housing sites: 1) Hire behavioral health direct service teams made up of qualified licensed and license-eligible behavioral health clinicians and non-masters level frontline staff to provide specialty behavioral health services, nursing support for individuals with complex medical needs, and milieu support with treatment and health promotion groups; 2) Train and onboard behavioral health teams, in coordination with BHS/WPIC, HSH and PHACS leadership; 3) Ensure behavioral health teams are well-trained in interventions for and access to mental health and substance use services, overdose response and prevention, harm reduction philosophy and techniques; 4) Provide administrative and clinical supervision for behavioral health staff; 5) Work with BHS/WPIC, HSH and PHACS leadership to implement system for responding to requests for behavioral health services; 6) Coordinate implementation activities with BHS/WPIC, HSH and PHACS leadership; 7) Collaborate with PHACS leadership and the San Francisco Health Network (SFHN) CalAIM team to implement ECM services, including training of staff and following ECM guidelines. b) Collaborate with participating DPH and HSH departments and contractors: 1) Coordinate implementation and ongoing services with BHS/WPIC, HSH and PHACS leadership; 2) Collaborate with BHS/WPIC and HSH on service coordination and quality improvement activities; 3) Attend regular program and system-related meetings. c) Provide Service Documentation, Data Collection & Reporting: 1) Ensure documentation in SFDPH EPIC of services and clients served and bill Medi-Cal as applicable; 2) Ensure security and confidentiality of data; 3) Share qualitative and quantitative data on regularly established basis. d) Implement and provide services with an approach grounded in racial equity, health equity, cultural humility, and harm reduction principles. 2. Modalities / Interventions A Scope of Services may include, but not be limited to, the following Modalities / Interventions: a) Consultation for staff of permanent supportive housing sites regarding behavioral health needs and interventions; b) Direct short-term behavioral health services with goals of assessing and stabilizing behavioral health needs, including delivery of specialty behavioral health services (billing to Medi-Cal); c) Linkage to ongoing care, and care coordination with existing providers and housing staff and ensure services are coordinated with other behavioral health, treatment and crisis services. d) CalAIM Enhanced Care Management (ECM) services. 3. Objectives and Measurements. Objectives and Measurements will be developed jointly during the contract negotiations. If acceptable Objectives and Measurements cannot be developed the City acting in its sole discretion will terminate negotiations and move to the next highest ranked Applicant.
