SERVICE ATTRIBUTES • Must be able to perform STAT deliveries of all blood product lines (RBC, platelets, frozen plasma, cryoprecipitate) • STAT deliveries occur within four (4) hours of ordering. • Effective, timely and consistent communication regarding stocking (inventory) levels and shipping delays for both routine and stat orders. • Must maintain high level of quality service including order fulfillment including but not limited to, product deliveries arrive on schedule, product arrives in satisfactory condition, timely and accurate reporting of reference lab results, timely resolution of problems, and timely resolution of problems and billing discrepancies. • Must provide means of immediate communication (e.g., phone numbers) CLINICAL REQUIREMENTS • Must provide blood, blood products and blood-related services to Health Services, as needed as designated by hospital blood banks. • Shall provide directed or autologous blood at the request or direction of a patient’s physician and Hospital’s Transfusion Medicine physician. • Shall provide the following special-order products for patients in clinical need: HLA phenotyped platelets, cross-matched platelets, HLA select or HLA A and B matched platelets, phenotype RBC units, leukoreduced, irradiated or CMV negative cellular products, rare donor products, deglycerolized RBC, saline washed RBC, granulocytes, Whole blood, and Liquid Plasma. • Meets requirements as stated in the CIRCULAR OF INFORMATION For the Use of Human Blood and Blood Components. • Possesses active registration or license (Federal, State and Local) for blood product distribution.
