Specifications include, but are not limited to: Behavioral Health Services (BHS), in partnership with San Diego Housing Commission (SDHC), are collaborating and partnering in a joint endeavor to provide behavioral health support services and Permanent Supportive Housing (PSH) to persons residing at two SDHC properties. SDHC-Valley Vista contains 190 PSH units, including two manager units at the property. SDHC- Kearny Vista contains 142 PSH units, including two manager units at the property. PSH units are a combination of studios, 1-bedroom and 2- bedroom units: • SDHC - Valley Vista: 156 1-bedroom and 34 2-bedroom units • SDHC - Kearny Vista: 108 studios and 34 2-bedroom units Contractor shall provide behavioral health supportive services and case management to the residents at Valley Vista/Kearny Vista SDHC property in accordance with the residents' behavioral health and housing needs to support residents' retention in the housing development, and access to mental health and substance use services. Behavioral health support services at the identified SDHC PSH development include screening, outreach and engagement, behavioral health assessment, case management, housing stabilization, crisis intervention, and care coordination to needed behavioral health and other social services. To ensure all residents receive the appropriate level of services, contractor shall provide two levels of case management services. • Light Case Management o To provide behavioral health support services and support housing stability to residents who do not require clinical case management. o A minimum of 1 face to face contact per month. o Contractor shall enter services into the Coordinated Entry System (CES) and the region's Homeless Management Information System (HMIS), as needed. • Clinical Case Management o For residents who are high risk, who need behavioral health screening, crisis intervention and care coordination to link to BHS services. o A minimum of 2 face to face contacts per month. o Contractor shall enter services into County's electronic health record(s). o Contractor shall enter services into the Coordinated Entry System (CES) and the region's Homeless Management Information System (HMIS), as needed. To assist potential residents (applicants) with the Project Based Voucher (PBV) application process for any vacant units.
