The California Department of Public Health (CDPH) is soliciting proposals from firms that are able to deliver six 20-hour Academy sessions, track attendance, conduct evaluation, and meet regularly with CDPH. The Contractor will be responsible for carrying out activities and documenting deliverables to achieve the goals outlined in Exhibit A, entitled “Scope of Work”. The Contractor shall conduct six 20-hour Academy training sessions (either virtually or inperson, and in English and Spanish), led by a Community Health Worker (CHW) facilitator to priority organizations, including federally qualified health centers, managed care plans, community clinics, non-profits, school-based health centers and community-based organizations conducting social services, asthma home visiting, or education services. Proposals must address all the services described in Exhibit A, Scope of Work. CDPH intends to make a single contract award to the most responsive and responsible firm offering the highest scoring bid. This procurement is open to all eligible firms and/or individuals that meet the qualification requirements, including commercial businesses, nonprofit organizations, State or public universities (including auxiliary organizations) and other entities.
