The County of Fresno on behalf of the Department of Behavioral Health (DBH) is requesting proposals from qualified vendors to provide an Adult Continuum of Care program for Assisted Outpatient Treatment (AOT) and Community Assistance Recovery and Empowerment (CARE) Act Services. Full-Service Partnership (FSP) including Outpatient (OP)/Intensive Case Management (ICM) services shall be provided to adults 18 and older with a severe impairment resulting from a diagnosis of serious mental illness (SMI)/Serious Emotional Disturbance (SED) as set forth in the California Welfare and Institutions Code (WIC), section 5600.3(a). The Continuum of Care shall provide comprehensive mental health, housing and other community-based supports for adults. Creating a Continuum of Care is for maximum flexibility to move identified persons served between levels of care, as clinically appropriate, with minimal disruption to services.
