Specifications include, but are not limited to: Alameda County Health Care for the Homeless (ACHCH) is a federally funded health center based in the Alameda County Health Care Services Agency (HCSA), whose mission is to provide comprehensive health care and housing services to improve the health of County residents who are experiencing homelessness.; ACHCH provides medical and behavioral health services in street and shelter settings, convenes a Countywide network of homeless service providers, provides training and technical assistance, conducts analyses of mortality data, and leads emergency response efforts for people experiencing homelessness (e.g., COVID-19 pandemic, smoke, and extreme weather conditions).; ACHCH provides services to more than 9,000 people annually. Approximately 15 percent receive care from ACHCH County-employed clinicians and staff; the remaining 85 percent receive care through ACHCH’s contracted network, which includes Alameda Health System, community-based federally qualified health centers and other providers of street and mobile health services.; 1. Design a strategic visioning and planning process (Estimate hours: 30); 2. Conduct Strengths, Weaknesses, Opportunities, Threats analysis (Estimated hours: 30); 3. Facilitate 4-8 stakeholder meetings with: staff, Commission, Community Consumer Advisory Board (CCAB), and HCSA leadership to support team-building, and strategic visioning and prioritization (Estimated hours: 140); 4. Produce an executive summary of actionable, strategic priorities for 2022-2025 (Estimated hours: 60)
