Contractor is to fabricate and repair dental removable prosthetic (i.e., Stayplates, Partial Dentures, Full Dentures, etc.) at the contractor’s laboratory. If Contractor has the ability to fabricate fixed prosthetic appliances, the scope of this service shall be included in the contract. Contractor will pick up and return cases at the clinic in which the treatment has been rendered. Contractor will provide an invoice with each case. Contractor will submit separate monthly billing statements for each clinic, clearly indicating the case and the clinic in which the case originated. Contractor shall confer with the treating dentist on any case for which there is a potential concern. Contractor shall be responsible for directing and reviewing the quality of all dental laboratory cases. Contractor shall report to the SMMC Dental Program Director any problems or concerns as they arise. Cases will be completed within the time frame agreed upon. It is imperative that a good billing and communication system be established from the onset of this agreement. Cases will be sent from dentists licensed in the State of California and from dental students. The students will be working under the supervision of a licensed dentist who is on faculty at the dental school and credentialed by San Mateo Medical Center.
