Program Description 1. Program Objective Adult long-term and transitional residential treatment services are intended to provide a stable, structured therapeutic home-like environment within a community. Selected Contractors will be providing either a long-term (up to 18 months) residential treatment facility or a transitional (up to 12 months) residential treatment facility which includes housing and mental health treatment geared towards the client attaining and maintaining community stabilization. The acute psychiatric hospital or locked adult residential facility treatment team, along with DBH’s discharge/placement team, will evaluate and determine if clients can benefit from continued structured treatment services to enhance their independent living skills that may aid in their successful transition back into their community. The DBH discharge/placement team will coordinate client referrals with the RFP selected Vendor and assist with placement into either the long-term or transitional facility providing the appropriate level of care. DBH will be the sole referral source through the term of the contract. 2. Program Requirements a. All Adult Residential Treatment services must follow the California Code of Regulations (CCR) Title 22- Adult Residential Facility, which provide treatment services listed within CCR Title 9 - Article 3.5 Standards for the Certification of Social Rehabilitation Programs including CCR Title 22 § 51341. b. All Long Term and Transitional Residential Treatment Programs must follow CCR Title 9 Section 531 (b and c) for Transitional Residential Treatment Program, and provide: i. Services as specified in either subsection (h) or (i) of section 541 which shall provide a therapeutic environment in which clients are supported in their efforts to acquire and apply interpersonal and independent living skills. The program shall also assist the client in developing a personal community support system to substitute for the program's supportive environment and to minimize the risk of hospitalization and enhance the capability for independent living upon discharge from the program. The planned length of stay in the program shall be in accordance with the client's assessed need, but not to exceed one (1) year; however, a length of stay not exceeding a maximum total of 18 months is permitted to ensure successful completion of the treatment plan and appropriate referral. The reasons for a length of stay beyond one (1) year shall be documented in the client's case record. ii. Services as specified in subsection (j) of section 541 in order to provide a 24-hour therapeutic residential setting with a full range of social rehabilitation services, as defined in section 532 of these regulations, including day programming for clients who require intensive support in order to avoid long-term hospitalization or institutionalization. The planned length of stay shall be in accordance with the client's assessed needs but under no circumstances may that length of stay be extended beyond eighteen (18) months. c. Provide and maintain a home like environment within a community that is also a fully functioning long-term or transitional facility providing residential treatment services for clients referred by DBH. d. Facilities will remain in compliance with a certification of Social Rehabilitation Treatment Services issued by the State of California Department of Health Care Services and a license as an Adult Residential Facility (ARF) from Community Care Licensing (CCL). e. Food Services: Meals will be provided on-site. The expectation is a minimum of three meals and two snacks daily. Some meals will be prepared by clients to assist in education of daily living activities. Snacks should be easily accessible with a focus on fresh produce. A monthly menu will be posted by the first of each month. f. Access to Phones/Outside Communication: Phone and mail access will be available, unless limited access is addressed in the client’s needs and service plan and agreed upon by the Office of the Public Guardian should the client be a Lanterman Petris Short (LPS) Conservatee.
