The County is seeking Responders to provide Services Coordination which may include access to workforce, education, and training programs, or other services needed to promote housing stability in supportive housing. Eligible uses of HHAP 2 funding for Service Coordination include: A. Wrap-around services: Providing a comprehensive suite of support services to address various needs, such as housing navigation, case management, and access to employment, education, and training programs. B. Housing navigation: Assisting individuals and families in finding and retaining permanent housing, including rental subsidies, landlord incentives, and move-in costs. C. Case management: Providing ongoing support and guidance to clients, including connecting them with appropriate resources and services. D. Access to supportive services: Facilitating access to employment development programs, mainstream benefits, disability benefits, food and other basic needs, and other supportive services. E. Cultural mentorship: Offering culturally sensitive support and guidance to individuals and families. F. Other services needed to promote housing stability: Any other services deemed necessary to help clients achieve and maintain stable housing.
