1. 1099-MISC Form, Blank, Copy B, 2-UP With Backer Instructions; Part #5108B or Approved Equal 1.1 Contractor shall provide blank 2-up forms for 1099-MISC with Copy B instructions printed on the back of each form. 1.2 Contractor shall provide in packs of 500 sheets (packaging shall be at Contractor’s discretion). 1.2.1 Contractor shall provide an estimated annual quantity of 3,000 finished paper size or roughly 6,000 forms with the perforation. 1.3 Sheets shall contain one (1) horizontal perforation, creating two (2) separate forms. 1.4 Finished paper size shall be 8.5” x 11”. 1.5 The finished paper weight shall be of 24 lb. bond paper or its equivalent of 60 lb. uncoated text paper. Contractor shall verify this paper weight requirement for each shipment/delivery. 1.6 Contractor shall print the most current 1099-MISC instructions on the back of each form. 2. Envelope for 1099-Misc 2Up; Part #7777-1 or Approved Equal 2.1 Contractor shall provide envelopes for 1099-Misc 2Up 2.2 Contractor shall provide in packs of 500 sheets (packaging shall be at Contractor’s discretion). 2.2.1 Contractor shall provide an estimated annual quantity of 6,000 envelopes.
