Specifications include, but are not limited to: A. Flyer/Promotional Materials 1. The Facilitator must develop a flyer and/or other promotional materials to recruit a minimum of 10 AI/AN community members to attend each of the 6 Cooking for Wellness classes (a target of at least 60 unique attendees in total). 2. Priority registration must be given to members of the AI/AN community with at least 75% of the participants identifying as members of the AI/AN community within the colonized borders of the U.S. 3. The attendees should come from all 8 Service Areas in Los Angeles County. 4. Any flyers and/or other promotional materials must include a copy of the LACDMH logo, the AI/AN UsCC logo, and the MHSA logo. 5. The flyers and/or other promotional materials are the property of LACDMH. 6. Any flyers and/or promotional materials should include the statement, “This Project is a Product of the AI/AN UsCC Subcommittee”. 7. The flyers and/or other promotional materials including those utilizing social media platforms must be submitted to LACDMH for approval prior to implementation. i. All social media posts must include information on where and how to access mental health services. ii. The LACDMH 24/7 Help Line must be included in every social media post (LACDMH 24/7 Help Line 800-854-7771), as well as the 988 Lifeline. iii. Facilitator shall be solely liable and responsible for any and all social media posts. Facilitator shall indemnify and hold harmless the County from and against any liabilities and costs arising from, connected with, or related to social media posts as part of this project. 8. Upon the request of LACDMH, the Facilitator must demonstrate that they fulfilled this deliverable by providing supportive documentation (e.g., receipts, contracts, attestations, etc.). 9. Payment is contingent upon completion of Deliverable activities (as listed above) and submission of all required paperwork. B. Pre and Post-Test 1. The Facilitator must develop a pre and post-test to be administered to attendees of the Classes to gather information on the level of knowledge gained by the attendees. 2. The pre and post-test must utilize a Likert Scale. 3. The pre and post-test must be submitted to LACDMH for pre-approval. 4. The Facilitator must submit copies of all completed pre and post-test to LACDMH. 5. The pre and post-test must include a copy of the LACDMH logo, the AI/AN UsCC logo, and the MHSA logo. 6. Upon the request of LACDMH, the Facilitator must demonstrate that they fulfilled this deliverable by providing supportive documentation (e.g., receipts, contracts, attestations, etc.). 7. Payment is contingent upon completion of Deliverable activities (as listed above) and submission of all required paperwork. C. Development of Schedule and Agendas for Cooking for Wellness Classes 1. The Facilitator will conduct 6 classes in total. Each class should be approximately 3 hours. 2. The Facilitator must develop the Agendas and all materials, written at a 6th grade comprehension level, for the 10 classes. 3. The Facilitator must provide LACDMH with a schedule of all of the classes in advance. 4. Each of the classes should focus on different topics, activities, and recipes. 5. Facilitator must gather input from members of the AI/AN UsCC subcommittee at one of the subcommittee’s monthly meetings, as well as the AI/AN community at large to determine the focuses for the Agendas of the 6 classes. 6. The Agendas must be relevant to the AI/AN population. 7. The Agendas and materials must be submitted to LACDMH for approval prior to implementation. All materials belong to LACDMH. 8. Upon the request of LACDMH, the Facilitator must demonstrate that they fulfilled this deliverable by providing supportive documentation (e.g., receipts, contracts, attestations, etc.). 9. Payment is contingent upon completion of Deliverable activities (as listed above) and submission of all required paperwork.
