The Cost Proposal should include the following:

Title Page – Title page showing the request for proposal number and subject; the firm's name; the name, address, telephone number, and e-mail of the contact person; and the date of the proposal.

Certification – A certification that the person signing the proposal is entitled to represent the firm, is empowered to submit the proposal and is authorized to sign a contract with the District.
