Specifications include, but are not limited to: A. Before placement for adoption, the Division shall compile and provide to the prospective adoptive parents a detailed Adoption Summary, including written health history and genetic and social history of the child which excludes information which would identify birth parents or members of a birth parent’s family. The detailed, written health history and genetic and social history shall be set forth in a document that is separate from any document containing information identifying the birth parents or members of the birth parent’s family. B. Contractor shall complete the Adoption Summaries and submit them to the appropriate Division of Children and Family Services (DCFS) designee, typically, the Adoption Supervisor, within fourteen (14) days of the referral to the contractor. C. Contractor shall ensure that the Adoption Summary includes one (1) face-to-face visit with the child and the placement provider. When age appropriate, likes, dislikes, wants, and needs must be included in the profile. The child should also be asked about the type of family he/she would like to adopt them and what they would want the family to know about them. For verification purposes, bidder must include a sample summary, demonstrating the approach to gathering information and preparing it for review and evaluation in report form. D. Contractor shall utilize the provided template from DCFS when completing adoption summaries. E. Contractor shall ensure quality assurance on the adoption summaries.
