Specifications include, but are not limited to: I-HELP is an effective, equitable and sustainable model. It engages not only those experiencing homelessness but local faith partners, non-profits, healthcare institutions and government agencies. Each night, individuals experiencing homelessness meet at a central intake point and are transported to the rotating house of worship where they access safe shelter. Participants, in partnership with a case manager, create a personalized service plan detailing the steps necessary to transition to a life of stability. Case managers also provide critical links to healthcare resources including substance abuse, behavioral health counseling, primary care and health screenings, employment and job training resources, and housing and benefits advocacy. The model helps optimize health and wellness through prevention, access to care, shared resources and advocacy. In the northwest valley this would be a new program and will provide services to a vulnerable and previously unserved population. Almost all individuals and families experiencing homelessness that access services at the Surprise Resource Center do not want to go to downtown Phoenix for shelter options but would access a local program if one existed. City of Surprise, through its selected Offeror/Contractor, shall comply with the following service requirements for the provision of emergency shelter: 1. Facility - Emergency Shelter Services a. Provide shelter services for homeless persons in the West Valley, including the Cities of Surprise, El Mirage, Youngtown and other West Valley communities including unincorporated Maricopa County. b. Operate one or more shelters with the CSBG funds awarded herein along with other public and private funds. c. Comply with applicable state and local zoning, fire, environmental, health and safety standards and regulations. d. Ensure that shelter facility is safe, secure, clean, in good repair, and building has proper ventilation and heating/cooling systems. e. Maintain a fire safety and emergency evacuation plan for clients, staff and volunteers. f. Ensure that appropriate training/access to first aid equipment and supplies are available in case of a medical emergency. g. Maintain adequate staff to provide for the supervision and wellbeing of clients at all times. h. Provide equal access per American with Disabilities Act to programs and services for individuals with disabilities in compliance with the Americans with Disabilities Act. Contractor shall provide reasonable accommodation for clients in the following categories: visual impairments, hearing impairments, mobility impairments and/or mental impairments. i. Contractor shall comply with Fair Housing Act.
