The Subrecipient shall: 11.1. Provide a home visitation program through the development of administrative, management, organizational systems and information, and referral networks to implement and manage a Health Start Program at the contracted site in targeted communities/service areas at risk for preterm and low birth weight births and high rates of infant mortality per the Health Start Policy and Procedure Manual (HSPPM): 11.1.1.
