Maintain all licenses and certifications, including but not limited to Clinical Lab Improvements Amendments (CLIA) and the College of American Pathologists Forensic Drug Testing program. 3.2 Perform all laboratory testing within the entire State of Arizona. 3.2.1 Provide testing sites throughout Arizona, with multiple locations available to the individuals to be tested. Testing services shall be available throughout the state of Arizona, to include rural as well as urban locations. 3.2.2 Provide a complete list of sites/locations relevant to the performance of this contract. 3.3 Drug test, including collection, confirmation, and medical review for: 3.3.1 Amphetamines: Amphetamine, Methamphetamine. 3.3.2 Cocaine Metabolite. 3.3.3 Opiates: Codeine, Morphine. 3.3.4 Phencyclidine. 3.3.5 Marijuana Metabolite. 3.3.6 Barbiturates. 3.3.7 Benzodiazepines. 3.3.8 Methadone. 3.3.9 Propoxyphene. 3.3.10 Heroin metabolite. 3.3.11 Fentanyl. 3.3.12 Alcohol 3.4 Charge the individual who requires the test at the time of testing. There shall be no cost to the State of Arizona for the testing. 3.5 Make available the test results via an online portal that ADHS will access upon email notification sent to the email address provided at section 8.1. The Contractor shall provide a secure cloud-based environment for storing PHI (including proof of an initial cybersecurity assessment certification and a plan for conducting annual cybersecurity assessment certifications), proof of AZRamp status, and a Business Associate Agreement to outline how PHI will be protected, including how PHI data will be encrypted at rest and in transit. 3.6 Provide a Certificate of Liability Insurance (COI) that exactly matches the Insurance Provision in the Special Terms and Conditions. A current and valid COI shall be maintained throughout the Term of the Contract. A Purchase Order to commence services will not be provided without the COI on file with the ADHS Procurement Office. 4. Tasks The Contractor shall: 4.1 Provide an agreed upon test panel, including confirmation, gas chromatography for all positive results and dilution and adulteration testing for the sample to be tested. 4.1.1 In the event of a false-positive result occurs, the supplier shall notify the individual and ADHS via email notification that a follow-up test is required. 4.2 Secure specimens at all times throughout the collection process, including chain of custody documents. 4.3 Keep positive samples in a locked freezer in their original containers for a minimum of one (1) year from the date the sample is taken.
