3.1. Provide an all-inclusive service cost to provide septic tank pumping, and disposal services, to include but not limited to; all required material, necessary equipment, containers, disposal fees, transportation, fuel surcharges, services, labor, benefits/payroll, travel expenses, insurances, workers compensation fee, and overhead. ADCRR will not reimburse any other expenses. 3.2. Be properly licensed by the Arizona Department of Environmental Quality for the service identified herein, see Attachment 4. Questionnaire. 3.3. Comply with the Arizona Department of Environmental Quality, Arizona Administrative Code (AAC) under Title 18, Chapter 9 requirements for services described herein. (www.azdeq.gov) see Attachment 4. Questionnaire. 3.4. Be in receipt of a Purchase Order is required prior to the scheduling of services. Services are not permitted without the issuance of a purchase order. 3.5. Be escorted to the specific service location(s), and upon completion of services, the facility’s Physical Plant Manager or authorized designee shall perform a visual inspection and written approval (e.g. signature on Contractor's service ticket or other document) prior to the Contractor departing the job site. 3.5.1. A copy of the signed service ticket or other document must be attached to each invoice for payment. 4. Specifications, Contractor shall: 4.1. Services shall include the pump out of contents, clean the baffles and tank, inspect the tank, and dispose of all liquid waste in a certified disposal facility in accordance with local, city, state and federal regulations. 4.2. All spillage of removed contents by the Contractor during services or transportation shall be the responsibility of the contractor and shall be immediately and properly cleaned by the Contractor in accordance all applicable Federal, State, County and local Government jurisdictions, laws, regulations, ordinances and policies, health, safety and cleanliness standards. 4.2.1. All costs associated with compliance to this section shall be borne by the Contractor. 4.3. A manifest for non-hazardous waste from a certified landfill shall be filled out and signed by an authorized contractor representative and kept by the contractor.
