The GCCL credentials are satisfactory completion of the following courses offered by the University of Arizona Cooperative Extension Onsite Wastewater Program. o National Association of Wastewater Technicians (NAWT) Inspector Training o Soil & Site Evaluation for Onsite Wastewater Systems o Introduction to Design of Onsite Septic Systems • Indicate if you are quali�ied to design conventional systems, alternative systems, or both. • Provide your State of Arizona Professional Engineering Registration Number and a statement that you possess knowledge and competence in the design and installation of onsite systems according to the Arizona Aquifer Protection Permit Program 18 A.A.C. 9, Articles 1-4 • Provide up to six (6) Approved for Construction Plan Sets and accompanying Construction Authorizations received within the period of July 1, 2024, through August 1, 2025. • Supply a customer reference list of up to six (6) onsite customers including the submitted designs, and to which County the designs were submitted. • Provide up to two (2) full sets of Approved for Construction documents for the submitted designs. • Indicate if you will provide the topography for alternative system projects or if this activity will be subcontracted. • Indicate that you will be able to submit a Notice of Intent (NOI) and complete submittal package within three (3) weeks of noti�ication of project selection and respond to a needs list within one (1) week of receipt. • Indicate that you will be able to perform a �inal inspection within one (1) day of the contractor’s request and all Discharge Authorization documents within three (3) days following the �inal inspection. SITE AND SOIL INVESTIGATORS • Indicate if you are willing and quali�ied to perform the Site and Soil Investigation, fully complete the Uniform Site Investigation Report Form (soil testing), and meet the quali�ications below. The site and soil investigation may also be subcontracted out by the Designer to person(s) meeting the following criteria. If subcontracted, the subcontractor’s quali�ications are to be submitted along with those of the Designer. • Training and registration for Site and Soil Investigators. o Arizona-Registered Professional Engineer, Arizona-Registered Professional Geologist, ArizonaRegistered Sanitarian, and/or certi�icate of training from a course recognized by the Arizona Department of Environmental Quality (ADEQ). o Submit current registration or course completion documents. • Currently listed on the Gila County Contractors List (GCCL) or, if not, supply equivalent credentials to those required to apply for inclusion on the GCCL.
