Duties to be performed as follows but not limited to: Screenings Direct Therapy Services and/or Tele-Therapy Services in accordance with state licensure requirements and within standards of professional ethics Evaluations Meetings for permission to evaluate, individual evaluations, evaluation reports and meetings to explain the results of evaluations to determine eligibility and developing of IEP’s Provide all testing materials Provide own equipment and materials to perform duties; MVESD16 will not be responsible for nor issue laptop computers or other equipment needed by the Contractor or Contractor’s therapists for remote work Design and implement Physical Therapy Services to assigned case load at assigned sites Provide monthly billing and student contact forms as requested by MVESD16’s Director of Exceptional Student Support Services; Contractor shall fully comply with all other state and federal requirements associated with provision of the services contemplated herein and not bill any other party (e.g. Medicaid, 3rd party insurance, or parents). Violation of this provision will result in termination of the contract. Agree to defend, indemnify, and hold harmless MVESD16 and its officials, agents, and employees from and against any and all claims, actions, suits, or proceedings of any kind brought against said parties as a result of any services performed by the Contractor under this agreement.
