Lead Based Paint sampling & testing and report shall comply with Healthy Home Grant requirements and all applicable Federal, State and Tribal Legislation and regulations pertaining to the Lead Based Paint, and shall also comply with the Lead Based Paint Poisoning Prevention Act and the Residential Lead-Based Paint Hazard Reduction Act of 1992 and 24 CFR Part 35, Sub-parts A, B, H, J, K, M, and R (IHBG & ICDBG). Lead Based Paint sampling and testing shall also be conducted per industry standards per the respective units. Reports shall also have all necessary information for use towards abatement of unit(s). • Survey shall include, but not limited to walls, ceilings, flooring, doors, window assembly and other building materials suspected of containing Lead Based Paint. • Surveys shall include, but not limited to, clearly marked maps of survey points with floor plan(s) included with the survey report. The report shall also include coordinated test results from the testing agency, site survey log, clearly described test locations and clearly described materials tested. The results, testing log, and descriptions shall be collated into a single report with survey location identification number corresponding to the floor plans. The report will also have a summary identifying the methodology used for testing and descriptions of areas and or materials with positive results. • Completed project abatement drawings & specifications shall include, but not limited to, the survey report, a scope of work, definitions, related references to governing code and regulations, any necessary plan drawings to locate any needed abatement materials, products, equipment, abatement methodology, containment procedures, prescribed containment areas, site deconstruction, waste disposal and chain of custody procedures. • Third Party Abatement Monitoring and Air Monitoring- In the event abatement is required, the Contractor shall provide an air sampling technician/professional to monitor the performance of the abatement work by the selected demolition contractor. The Contractor’s monitoring shall include the abatement contractor’s preparation of Lead Based Paint containing materials for removal. • Close out Documentation- An Abatement Report (Close out Documentation) shall be prepared which shall provide documentation of abatement activities and air monitoring results. The report will contain report of air monitoring samples collected during the abatement phase of the project and supporting documentation; in addition to any abatement contractor supplied documentation, including but not limited to worker certification. • Selected Contractor shall sample/test all units constructed prior to 1978.
