1. Physical Examinations: Physical examinations shall be conducted by a health care professional in accordance with the DOT Physical Examination Form, in addition to physicals for fit for duty. Non DOT physical examinations shall be conducted by a health care professional for required nondriving staff. As required by DOT, Physical examinations must be conducted every 24 months from the date of the initial physical examination unless a shorter time is specified by the health care professional who administers the physical examination. All chain of custody, physical exam reports shall be emailed to Transportation Field Operations Manager for DOT and Non DOT staff. Contact information will be furnished upon award. 2. Controlled Substance/Drug Testing: The purpose of the contract is for members of the district to be in compliance with the Omnibus Transportation Employee Testing Act of 1991, the Arizona Department of Administration Minimum Standards for School Buses and School Bus Drivers, AAC, Title 13, Chapter 13 and other associated regulations. Respondents to this RFQ should be familiar with the requirements of the AAC. Terms used herein are defined in Section 49 CFR 382.107. The following requirements are from the U.S. Department of Transportation: 1. Tests shall be conducted using urine split samples with a 30-ml primary sample and 15-ml split sample. A screening test will be performed on the primary sample and, if positive, a confirmation test will be performed on the split sample. 2. A clear and well-documented procedure will be maintained for collection, shipment, and accessioning of urine samples. 3. A standard U.S. DOT drug testing custody and control form is required. 4. Collection sites must have all the necessary personnel, materials, equipment, facilities and supervisors trained to provide for the collection, security, temporary storage, and shipping of urine specimens to a certified drug testing laboratory. 5. Collection site personnel must be trained to carry out this function or be a licensed medical professional or technician provided instruction to carry out this function. 3. Alcohol Testing: The following requirements are from the U.S. Department of Transportation: 1. Tests will be conducted using an Evidential Breath Testing Device (EBT) that appears on NHTSA’s Conforming Products List. 2. The EBT must be operated by a Breath Alcohol Technician (BAT) who has been certified and trained to proficiency in the operation of an EBT. 3. The test site must afford visual and aural privacy to the individual being tested. 4. A required testing form and/or logbook as prescribed by DOT must be used. 5. Test site must have available a documented receipt upon each completion of shift for the purpose of on-site audits. 4. Collection Sites: 1. The contractor must assure that the collection sites and procedures meet minimum industry standards to ensure secure, confidential sample collection and chain of custody procedures. 2. Regular hours of operation for transportation staff range for 5:00 a.m. to 7:00 p.m. Exceptions are field trips and sport activities since these events can be after regular hours of operation. 3. Preference will be given to respondents that can meet the flexible hours of operation. 4. All collection site locations must be listed. Travel time will be part of the evaluation. 5. The employees will have a maximum wait time of 30 minutes. 6. The drug and alcohol screening will be available until 7:00 pm and then on an on-call basis after that time. 7. On site collection services will be provided for random drug and alcohol testing. Travel time to be included. 5. Record Keeping on Testing: 1. Federal Law (49 CFR 382.403) requires that an annual calendar year summary be prepared in regards to the testing program. It is the intention of the district that the contractor will prepare this summary. Records shall be available to the district within 8 working hours of request. 2. Federal Law (49 CFR 382.401) requires retention of records related to the alcohol and controlled substance/drug testing. The district expects that the contractor will retain for the required period those required records that pertain to the collection process and those required records and those records would be available upon request by DOT for audit purposes. 3. The District would provide a CDL employee list for the contractors use in generating a random selection pool.
