Specifications include, but are not limited to: 1.1 Customers are referred and/or directed by the Court to register and complete counseling programs on an as-required basis. The Counseling Services most commonly consist of the following: • Alcohol/substance abuse screening and subsequent treatment (that primarily involves driving under the influence “DUI”); • Violence intervention (that primarily involves domestic violence); • Anger control; • Life skills; • Parenting skills; • Home detention; • Other as determined by referring Court; 1.2 The Customer is liable for all payment to the Contractor.; 1.3 The services will be provided on an as-required basis and the City does not guarantee a minimum amount of Services during the term of this Agreement.; 1.4 The Customer record shall be maintained alphabetically, by last name. The Customer record shall include the Customer signed invoice of Services and Price Sheet.; 2.1 Contractor shall have a current license issued by the AZ Department of Health Services, Office of Behavioral Health Licensing, for domestic violence and substance abuse counseling.; 2.2 The Contractor shall have the ability to conduct intake screening and treatment classes at the Contractor’s facility.; 2.3 Treatment classes must be available to Customers on at least two (2) evenings per week after 5:00 p.m. The availability of classes on the weekends is preferable, but not required.; 2.4 The Contractor will notify the Court of all non-compliances and all successful completions.; 2.5 The Contractor must maintain all current State and City licenses for the operation of its business throughout the term of the Agreement and shall provide proof of licensing and appropriate certification upon request.
