Specifications include, but are not limited to: 4.1. The University is seeking Proposals from qualified firms to provide Clinical Laboratory Services to the University’s Campus Health Services. Services will include Standard Laboratory tests and Cytology and Biopsy tests. 4.2. Proposer to be a health insurance contract provider, to include but not limited to the following: 4.2.1. Aetna. 4.2.2. Blue Cross/Blue Shield (HMO and PPO). 4.2.3. United Health Care. 4.2.4. CIGNA. 4.3. Proposer to provide an in-house Phlebotomist year-round, to include but not limited to the fall, winter, spring and summer semesters, at no hourly charge to the University. The University to be billed for each phlebotomy procedure performed by the in-house Phlebotomist. 4.3.1. Semester schedules Important Dates & Deadlines. 4.3.2. The Successful Proposer will be required to sign a Business Associate Agreement Business Associate Agreement as required by HIPAA which will be incorporated into the resulting RFP Contract.
