1. Individual Case Management Activities a. Develop and distribute a Welcome Packet to all new residents upon receipt of the SC23 Property Staff to Service Coordinator Referral Form. b. Offer to complete an Independent Living Needs Assessment (ILNA) for each new resident upon move in. The ILNA will be offered/updated annually. If the resident refuses to participate in the Service Coordination Program, the Service Coordinator will document the refusal using the SC22 Refusal of Recommended Services or Assistance and place in the resident’s file. Forms involved in the ILNA, for both new residents and annual updates for existing residents are as follows: i. SC23 Property Staff to Service Coordinator Referral Form ii. SC01 Confidentiality agreement iii. SC02b Consent to Release Information to AHFC iv. SC04 Resident Intake v. SC06 Resident ADL Status vi. SC08 Service Management Plan vii. SC13 Incident Report (if applicable)
