Z-Fold Requirements: Perforations: • All to be extraction perforations: Two full length perforations on each side measuring 1/2” from each edge. Six full width perforations from the top of the form to be ½”, 4-11/16”, 5”, 8-13/16”, 9-5/16” and 9-13/16”. • Form must fold to 81/2” x 4 11/16”. • Form must contain detailed opening instructions in PMS 285 Ink. • Backer Ink: Blue 285 security tint/pantograph on top and bottom panels; 285 blue 50% screen on middle panel. • Printed Guide Bar front and back on left side in the middle of the form. • Shelf life of 12 months including sealant minimum. Packaging • 4 reams per box. • 500 forms per ream. • Each ream shrink-wrapped Freight: Bid is F.O.B. destination. Any freight charges must be included in the bid prices. Quantity: The exact quantity of purchases for the item listed is 800 thousand. The Division of Purchasing does not guarantee that the state will buy any amount. Orders will be placed by Department of Revenue and will give complete shipping instructions. Delivery time frame: All items ordered must be delivered to the "ship to" address shown on the PO (Purchase Order) within six (6) business weeks of vendor's receipt of order. Default: When a vendor defaults on a contract/purchase order, the State can procure the goods or services from other sources and hold the vendor responsible for any excess in price/handling. The vendor may be barred from bidding for an indeterminate period. Unit of Measure: The unit of measure indicated is to be bid, or it could result in rejection of that line item. The unit is “M” or 1000. Requested information: Any additional information requested from a vendor must be furnished within five (5) days from receipt of request. Samples: Samples may be requested for evaluation and must be furnished without charge to the State.
