A. Service Delivery ● Provide PT services (direct, consultative, and collaborative) as required by students’ IEPs or 504 Plans. ● Deliver services in school settings such as classrooms, therapy areas, gyms, cafeterias, playgrounds, and hallways. ● Attend and contribute to eligibility, IEP, and 504 meetings as needed.d ● Collaborate with staff and families to promote student access and success. B. Assessments and Documentation ● Conduct evaluations and reevaluations using valid assessment tools. ● Generate written reports aligned with AAC and IDEA requirements. ● Complete session notes, Medicaid documentation, and progress reports ● All documentation must be recorded and stored in a secure, shared, internet-based system to which designated school staff have real-time access.
