Light Installation Madison Co DHR: One-time installation only for the interior of the building.• Madison County DHR has 85 lights currently out. Each fixture requires two fluorescent light bulbs:o 1st Floor: 50 fixtures (100 bulbs)o 2nd Floor: 27 fixtures (54 bulbs)o 3rd Floor: 8 fixtures (16 bulbs)• Remove existing fluorescent ballasts and install new fluorescent lamps.• Install new rapid-start electronic ballasts as needed.• Assess and resolve the non-functional stairwell fixture that remains out despite the new ballast installation that was completed in January 2025.• Ensure all necessary wiring, fixtures, and components are properly installed for full functionality—not just bulb replacement.• Cleanup of work areas after installation.Additional Requirements:Installation locations and exact quantities shall be determined during the site visit. Materials shall be supplied by DHR Madison Co. Office.• Unit price shall cover: installation, ballast and bulb removal, and cleanup.• Debris, old ballasts, lamps, and bulbs shall be disposed of off-site.• Verify with POC that supplies are on-site before scheduling installation.• Quote shall include all costs, including trip charges, mileage, supplies, labor, permit fees (if applicable), lift trucks, and any additional amounts necessary to complete the work.• All vendors shall have a current license on file at the time of award and installation.• All work shall be completed in one trip and finished by March 21, 2025.Payment terms:Net 30 paid in arrears after completion of service. No invoicing prior to completion of service.
