"9-Panel Drug Screen at Third-Party Site (Per Test) (Includes supplies and forms, centralize d billing and administration, random selection, cost of drug screen, and MRO service.)"; 5.) "Cannabidiol (CBD/THC) Ratio Screen (Per Test) (Includes supplies and forms, centralized billing and administration, cost of drug screen, and MRO service.)" The supplier’s testing laboratory must possess and maintain accreditation by: a. Clinical Laboratory Improvement Amendments (CLIA) in Urinalysis, and b. College of American Pathologists (CAP).
