Sexual Assault Evidence Collection Kit (BID) Sexual Assault Evidence Collection Kit that meets the following minimum specifications.Sexual Assault Evidence Kit Requirements3) Each kit box must be 10-1/4” length x 6-3/4” width x 2-3/8” height, and printed as per prototype.a. Kit boxes made of a minimum thickness of 0.045 regular 50 white wood board, corners stayed with a 7/8” wide 90 lbs. tan head seal stay tape.4) Kit boxes are to be pre-sealed by the manufacturer via the box top integrity seal in tabs and box top sealing flaps with Olympic 1330 water-soluble tamper-evident adhesive.5) Swab boxes are to have checkboxes for oral, DNA reference sample, genital, penile, vaginal, perianal, and rectal printed on the sides.6) Each kit box must contain the following items:a. 1 each, surgical face mask.b. 1 each, Instruction Sheet, printed front and back on 8-1/2” x 11” white stock.i. ADFS will provide text for the instruction sheet upon receipt of an award.c. 1 each, Step 1-Sexual Assault Information form printed on 8-1/2” x 11” 3-ply NCR type paper.i.
