Frequency of service: bi-monthly during the months of March thru October and monthly during the months of November thru February - approx. 20 services - billed at $xxx per service. QUOTE SHOULD BE SUBMITTED AS A PER SERVICE AMOUNT. (This amount will be adjusted to the number of services listed.) Awarded vendor to supply all chemicals, materials and/or equipment needed to perform these services. Subcontracting is strictly prohibited and will result in the termination of the contract. The person(s) performing this service must be the owner or a legal employee of the awarded vendor. Any damages to grounds, lawn, vehicles, and/or building during the performance of these services will be solely the responsibility of the awarded vendor. In order to properly quote this service, a site visit is mandatory. Please note that any quotes received from vendors that did not attend the site visit will NOT be considered.
