Specifications include, but are not limited to: Organizations receiving this award will be expected, at a minimum, to perform the following activities: Coordinate, implement, and cross-promote statewide medical home best practice activities; Prioritize equity and actively incorporate families, community professionals, health care providers, and healthcare systems into program planning; Collaborate with other health groups and networks to align, share, and incorporate medical home messaging and program initiatives; Provide related technical assistance to other agencies, including tribal health clinics, working on medical home; Create, maintain, and promote statewide curricula for multiple stakeholders to increase knowledge and skill for implementing medical home concepts; Create relationships, communications, and trainings that promote and advance medical home concepts with families, community professionals, health care providers, and health care systems, including tribal staff
