Specifications include, but are not limited to: Mandatory Requirements 4.1.1. One Contractor shall provide both the Short-Term Disability (STD) & Long-Term Disability (LTD), and Voluntary Long-Term Care Products pricing option plans. Provide a self-funded and fully-insured pricing options. 4.1.2. Short-Term and Long-Term products shall have an aggressive return to work programs with provisions that should employee refuse to participate shall result in loss of benefit. 4.1.3. Contractor shall provide W-2’s to employee for any fully insured portion(s) of the plan. 4.1.4. Contractor shall make all claims determinations and calculates benefit payments and then advise PWCS of the amount of benefit. 4.1.5. Short Term Disability shall not be less than 60% of gross pay and will be offset by sick leave. 4.1.6. Programs benefits levels must be at least equivalent in benefit as the current Virginia Local Disability Program. 4.1.7. Contractor shall make available necessary, appropriate and high-quality benefits program. 4.1.8. Contractor shall provide to all new hires working 20 hours or more a week. 4.1.9. Contractor, if possible, to provide a voluntary long-term care plan to assist with the cost, at a minimum, of the following: • Care in a nursing home or hospice • Assisted living facility • Community-based care • Home healthcare services • Informal care-giving • Alternative or transitional care
