Specifications include, but are not limited to: The Contractor shall evaluate the following at the individual, program, community services board (CSB) and private provider, regional, and statewide levels: 1. Whether individuals’ needs are being identified and met through person-centered thinking and planning (including building on the individuals’ strengths, preferences, and goals); 2. Whether integrated service options are available, accessible and being provided in the most integrated setting appropriate to the individuals' needs and consistent with their informed choice; 3. Whether individuals are supported to maximize opportunities for integration in all aspects of their lives (e.g., living arrangements, work and other day activities, access to community services and activities, and opportunities for relationships with non-paid individuals; 4. Whether providers are ensuring the sufficiency, accessibility, and delivery of quality services to effectively meet individuals’ needs in integrated settings; 5. Whether Provider quality improvement and risk management strategies are adequate and implemented effectively; 6. Whether Providers are meeting the intent of the HCBS settings requirements for individuals, as applicable; 7. Whether Support Coordination agencies are meeting SA requirements; and 8. Identification of barriers impacting individual achievement of outcomes at the CSB, provider and system level and recommendations to improve practice and the quality of services.
