Specifications include, but are not limited to: 1. Provide general supervision of SEVTC’s two full-time Licensed Physical Therapist Assistant (LPTA) in accordance with the state and CMS standards and regulations. 2. Provide complete physical therapy evaluation for each individual admitted to SEVTC within 72 hours. 3. Complete assessments, develop Physical Management Plans, prepare referrals and complete all related documentation. 4. Prepare an annual physical therapy evaluation of each individual of SEVTC who is receiving physical therapy services. Contractor shall complete the annual review at least one (1) week prior to the individual’s annual review meeting 5. Work on-site at SEVTC, or as otherwise agreed upon with the SEVTC Clinical Director or her designated representative. Consultations will be documented by signed consultation notes in the resident’s medical record. 6. Conduct themselves in a professional manner at all times and shall not behave in such a way so as to disrupt the general management of the Purchasing Agency where assigned. The Physical Therapist shall comply with all Purchasing Agency regulations, administrative policies and procedures. The Physical Therapist shall comply with instructions and requirements specified by the Purchasing Agency to coordinate physical therapy services
