Specifications include, but are not limited to: VCU Ryan White Program’s, “The Program”, overall objective is to obtain a Contractor, or multiple Contractors, to provide eyeglasses to our program’s patients in and around the Central Health District in the Commonwealth of Virginia. This Contractor should be capable of providing eyeglasses for referred patients eligible under The Program, delivering appropriate confirmations and invoicing, and appropriately price services to keep in The Program’s $20,000 annual budget. 1. Patient Referrals The Program will only refer eligible patients to The Contractor for the provision of prescription eyeglasses. Patients will not need to provide proof of eligibility during appointments scheduling with The Contractor. The fully-completed VCU Ryan White Programs Eye Health Referral form, see appendix D, and prescription for eyeglasses should verify eligibility. 2. Scheduling Appointments Eligible patients and assigned medical case managers from The Program will be responsible for scheduling an appointment date and time with The Contractor. In the event the appointment needs to be rescheduled, The Contractor is responsible for contacting the patient and assigned medical case manager. If a patient fails to arrive for the scheduled appointment, The Contractor is responsible for notifying the assigned medical case manager on the same business day. 3. Collaboration The Contractor or its authorized representatives shall meet annually with The Program to discuss all services during an onsite visit. The Contractor and The Program may mutually establish routine procedures for handling the services described herein. Such procedures are subject to The Program’s approval. The Contractor must be able to provide the following prescription lenses: a) Single vision b) Line bifocal c) Standard progressive d) Short progressive The Contractor will include the following for each patient in accordance with the fixed rates outlined under the “Budget and Payment” section: a) Up to two (2) pairs of eyeglasses (lenses and frames) annually b) Additional paraphernalia as requested by Contractor, based on medical necessity, and approved by the Program
