1. Perform Pre-Employment and Employee Physical Examinations on an as needed basis in accordance with standards prescribed by the American Medical Association. Adhere to American Correctional Association (ACA) standards, the DOC’s Directives and Operating Procedures (Operating Procedure 102.5 – Attachment 1) for Staff Medical Screenings and Physical Examinations, and all Federal, State, and Local Laws. 2. Ensure staff performing physicals meet all State, Federal, ADA, and HIPAA Laws & Regulations, and be qualified in accordance with the qualifications and licensing requirements of Chapter 8 Title 54 of the Code of Virginia 1950, as amended. 3. Be a licensed medical provider who is licensed in the Commonwealth of Virginia. 4. If requested, all licenses and certifications must be submitted for review prior to services being performed. 5. Determine the type of physical examination required based upon the applicant’s health history and responses to Employee Medical Evaluation – Employee Physical Examination Screening Form (Attachment 2) in addition to the applicant’s physical condition. 6. Complete the Licensed Examining Physician Report (CO-25, Attachment 3), Physical Training Requirements for Correctional Officers (CO-25A, Attachment 4), and Employee Medical Evaluation (Attachment 2) and submit them the day of exam completion to the DOC Human Resource contact specified at each hiring event. These evaluations shall be completed for Probation Officers, Correctional Officers, Food Services workers, and a variety of other facility staff. 7. Refer patient to his/her personal medical provider should any clinical concern arise in which the service provider is concerned about the patient’s health. The personal medical provider will then address the medical concern and complete a second pre-employment physical. 11. Perform services with care, skill, and diligence in accordance with the applicable professional standards currently recognized by such profession and shall be responsible for the professional quality, technical accuracy, completeness, and coordination of all findings. 12. Provide services on an as needed basis. 13. Bill the VADOC Human Resource Department accordance with the type of physical performed according to the Current Procedural Code (CPT Code Z02.1).
