Specifications include, but are not limited to: Provide services as detailed in 1 through 15 for up to 20 hours, every month at Fluvanna Correctional Center for Women. Services The Provider shall submit an accounting of hours worked each month to the facility’s Health Authority. The Provider shall prepare an invoice and bill the Department for the hours worked, up to the required total. Services provided in less than full hour increments shall be invoiced and paid in one-tenth of an hour increments. Lunch breaks shall be unpaid, and time taken must be deducted from the total month hours. If hours per month reach 20 and additional hours of service are required, the specific number of additional hours must be approved in writing by the facility’s Health Authority before services are provided or the Provider will not be paid for said hours. If service hours exceed the negotiated per month hours, any additional hours of service shall be approved in advance by the facility’s Health Authority and shall be paid at the negotiated rate. In lieu of an hourly reimbursement rate, the provider may agree to bill fee for service through Anthem HealthKeepers. Provide reasonable and customary conservative care treatments, including but not limited to foot examinations (diabetic and problem-focused non-diabetic issues), routine foot care (palliative trimming or debridement of toenails, paring of calluses and other benign skin lesions), and formulation of a treatment regimen for other acute, chronic or non-emergent foot and ankle conditions. Provide services in approximately 4-hour or 8-hour blocks of time, scheduling approximately three patient visits or treatment sessions per hour. This should allow adequate time for visits and/or treatments. Complicated issues may need to be staged over multiple sessions. The Provider and the Health Authority will establish a mutually agreed upon schedule.
