Specifications include, but are not limited to: A. ALL SWORN LAW ENFORCEMENT, TO INCLUDE ADMINISTRATIVE, FIELD AND K-9 TEAM (estimate 10 – 15 per year): all designated personnel will require the following tests as requested by the agency upon original appointment into position or as needed to ensure fitfor-duty eligibility requirements: a. Tests a. Complete Medical History b. Complete Physical Examination c. Pure Tone Audiometry (tested in a hearing booth) d. Pulmonary Function Test and Interpretation e. Chest X-Ray –(PA/Lateral) with review by a Radiologist f. Urinalysis (by dipstick methodology) g. Vision Test (tested with a Titmus vision machine) h. MRSA Screening i. Drug Screening b. Specialized testing or certification a. OSHA Respirator Certification b. CCI Physical Capacities Form c. Special examinations for schools/certifications/activities shall include a complete medical history and a complete physical examination. The examination form for the school/certification/activity identifies additional medical screenings and/or tests that must be completed. The physician may conduct additional tests as needed to ensure the applicant is able to participate in the school or activity or is fit for certification. Medical screenings/test may include one or more of the following: a. Vision, tested with a Titmus vision machine. b. Pure Tone Audiometry, test is conducted in a hearing booth. c. Pulmonary Function Test and Interpretation d. Chest X-Ray –(PA/Lateral) with review by a Radiologist (Alignment) e. OSHA Respirator Medical Evaluation f. EKG g. Echocardiogram h. Exercise Stress Test i. Coronary Calcium Scoring j. Urinalysis (by dipstick methodology) k. PPD d. Any blood analysis required will be drawn and submitted under the DWR account number. The DWR HR Officer will receive the Lab results and forward them to the Contractor. B. PRE-EMPLOYMENT MEDICAL EVALUATIONS (estimate 35 – 40 per year) – All Sworn positions (Probationary Conservation Police Officer and Conservation Police Officer (all ranks and specialties)) will require the following prior to DWR employment start date: a. Complete Medical History b. Complete Physical Examination c. Pure Tone Audiometry (tested in a hearing booth) d. Pulmonary Function Test and Interpretation e. Chest X-Ray –(PA/Lateral) with review by a Radiologist f. Urinalysis (by dipstick methodology) g. Vision Test (tested with a Titmus vision machine) h. MRSA Screening i. Drug Screening j. DWR may request that Pre-employment Examinations be conducted on up to thirty (30) employees on the same day. The Contractor will be given twenty (20) days’ notice to schedule such examinations for a large group. C. CIVILIAN MEDICAL EVALUATIONS (estimate 1 – 5 per year) - Dispatcher applicants sent as needed to ensure fit-for-duty requirements. 1. Pure Tone Audiometry (tested in a hearing booth) 2. Vision Test (tested with a Titmus vision machine) 3. Weight and Waist measurement 4. Blood Pressure Check 5. Urinalysis (by dipstick methodology) 6. EKG 7. Chest X-Ray – (PA/Lateral) with review by a Radiologist
