2.1.1.a The awarded vendor must be able to produce CII Prescription Forms in their current design and format to prevent confusion regarding authorized prescriptions. 2.1.1.b The minimum order quantity is to be one (1) pad consisting of fifty (50) prescription forms. All prescriptions must be distributed in sets of fifty (50) prescription forms. 2.1.1.c Pads will consist of forms grouped in a stack with control numbers running from low to high. Forms are to be bound in such a manner to allow the forms to remain securely in the pad until use and to remain intact on removal. Pads will include a cover for front and back of a stock to be agreed upon by vendor and TSBP. Prescription forms are to be 4 ¼” x 5 ½”. Pads will include instructions to the prescriber for use of pads. Pads will also include instructions for re-ordering pads. All instructions will be provided by TSBP. Instructions are subject to change at any time by TSBP. The prescription pad and its packaging must not contain information concerning any pharmaceutical product or other advertising or solicitation unless expressly approved by TSBP. Any changes must have prior approval from TSBP. 2.1.2 Printed Security Features The prescription form shall incorporate the following security features: The prescription form will contain a blue overlay that will void itself when copied by means of a hidden word feature. The ink will bind to the paper so that toner cannot be lifted without damaging the prescription form. Each prescription form will have a control number in a format determined by TSBP imaged or printed variably, in a horizontal direction. TSBP will determine the format of the control number. Each prescription form will incorporate a State of Texas seal as a background image. Each prescription form will include a thermochromic ink feature as indicated on current prescriptions to validate its legitimacy. Each prescription form will void itself if chemicals are used to modify any writing on the prescription. Each prescription form will include bar code(s) that contain data fields mutually agreed upon by the vendor and TSBP, including but not limited to: prescriber DEA number, control number, license number, or other numbers to be used in verification of prescriptions. Each prescription will include a watermark image within the body of the paper that cannot be copied, duplicated, or scanned. 2.1.3 Sample Evaluation 2.1.3.a Proposers must provide samples of a prescription pad to include all of the security features described in Section 2.1.2. 2.1.3.b Due to the highly sensitive nature of this work, a PDF file to print the sample pad will only be provided on request. Proposers are only authorized to print samples for purposes of this RFP.
