Specifications include, but are not limited to: pecifically include information about: a. The storage location proposed to house TTUHSC medical records. b. Technology used to inventory, locate and pull TTUHSC medical records. c. Moving TTUHSC records to the storage location. Service Requirements: 1. Move, store, inventory, locate and pull medical records as requested by TTUHSC. 2. Climate controlled, fire controlled protected and secure facility. (storage facility must meet environmental, security and fire detection standards listed in NFPA 232 including intrusion alarm systems.). 3. Monthly reports of all activity including records transferred to and from the storage facility. 4. Located in Lubbock and able to deliver charts on a daily basis. 5. Strict control procedures to safeguard confidential medical records. 6. Background checks as a condition of employment for all personnel that handles TTUHSC medical records. 7. Provide HIPAA training for all employees that handle TTUHSC medical records. 8. Monthly status meetings to review the Services provided and any issues. 9. Business continuity plan for any disaster (i.e. disruption of computer system, hacking, fire, etc.). 10. Protections for TTUHSC against sale of business or bankruptcy. 11. Satisfy the standards for personal health information contained in federal and state statutes and regulations including without limitation, any regulations promulgated under the Health Insurance Portability and Accountability Act of 1996 “HIPAA”. 12. TTUHSC requires access to inspect facility(ies) at any time. 13. TTUHSC requires access to pull records at any time. 14. Vendor must sign a TTUHSC Business Associate Agreement (See Appendix A). 15. Optional: On site shredding services.
