Specifications include, but are not limited to: Task 1: Provide high quality, comprehensive family planning clinical and education services to individuals of reproductive health age, in accordance with state and federal Title X guidelines and requirements. The Sub-Recipient will be responsible for providing high quality, comprehensive family planning clinical and education services, as outlined above in “Project Description” and further described in OPA clinical recommendations document “Providing Quality Family Planning Services” (link on page 2), to individuals of reproductive health age. The Sub-Recipient will be required to: Provide family planning clinical and education services to projected minimum target number of individuals of reproductive health age. Provide family planning clinical and education services to all individuals of reproductive health age who present for services regardless of age, gender, sexual orientation, disability, citizenship status, or residency. Provide comprehensive family planning clinical and education services to the targeted population for the full project year even if it has met the target number of clients to be served. Provide a broad range of acceptable and medically approved family planning methods. Implement policies and protocols that clearly incorporate state and federal Title X guidelines and requirements. Submit Family Planning Encounter Record (FPER) data to the Region I Title X Data System. Conduct monitoring and evaluation of services provided and required data/documentation. Task 2: Engage in outreach activities designed to increase community awareness about the availability of services. Planned outreach activities serve to facilitate community awareness of and access to family planning services. The Sub-Recipient will be required to: Establish and implement planned outreach activities whereby their services are made known to the community. Coordinate with other institutions (i.e. schools), as well as other community and social service agencies to inform potential Family Planning clients about available services. Develop evaluation tools to measure the impact of proposed outreach activities. Report outreach activities during the Annual Operations Review.
