Specifications include, but are not limited to: The Rhode Island Department of Administration/Division of Purchases, on behalf of the Rhode Island Department Behavioral Healthcare, Developmental Disabilities and Hospitals (“BHDDH”), is soliciting proposals from qualified firms to provide Projects for Assistance in Transition from Homelessness (PATH) services to assist individuals experiencing homelessness with serious mental illness (SMI) or co-occurring SMI and a substance use disorder (COD) in securing safe and stable housing, improving their health, and living a self-directed, purposeful life. The successful applicant will be expected to implement services to literally and chronically homeless adults in accordance with federal and state PATH grant requirements. These requirements include: • Outreach including street outreach • Screening and diagnostic treatment services • Case management services • Supportive and supervisory services in residential settings • Referral for primary health services, job training, educational services, and relevant housing services • Housing services
