Specifications include, but are not limited to: 1.Post-Offer Physicals to Include:a.Basic physical examinations.i.Medical history - Offeror is responsible for obtaining a medical historyquestionnaire to include baseline information, interval changes in healthstatus, and known potential occupational exposure.ii.Vital signs (temperature, pulse, respiratory rate, and blood pressure (BP))iii.Early identification of pre-disposing health conditions/factors;iv.Provision of medical instructions to employees to assist in the correction ofidentified pre-disposing health conditions/factors and actions needed toprevent the development of identified diseases;v.Communicate the presence of significant medical conditions identifiedduring the exam that would affect an employee’s fitness for duty; and.vi.Psychological Screening and Interpretationvii.Hemoglobin A1C – when diabetes has been documentedviii.When abnormalities are detected during the exam that are potentiallyindicative of the presence of a health condition, the physical exam must be carried through until a diagnosis is confirmed or ruled out. Employees mustbe referred to their own personal physician for follow-up of all otherpre-disposing health conditions; andix.Other miscellaneous services as identified by an individual agency.x.Coordination and review of position functional capacity evaluation, whenrequired as part of the post-hire evaluation process.b.Drug test/Urinalysis (cost per test, if applicable)i.10-panel rapid drug testii.Provide the County with copies of any new Federal and/or Stateregulations dealing with drug and alcohol testingc.Medical Review Officer (MRO) services (non-DOT)d.Audiometric Hearing test (non-DOT safety sensitive positions)e.Vision screeningf.Tuberculosis (TB) testing for specified high-risk operations employees
